If your loved one has passed away, or is nearing the end of their life, please don’t feel you have to wait to call us. Arrangements can be made at your pace, either before or immediately after someone passing away.

We are here to help, offer guidance and take away the worry or any doubts you may have. It’s never too early to talk.

Depending on the circumstance, please select from the choices below for more information on what to do for your loved one.

Passed Away at Home

The next steps to take if your loved one has been poorly and was expected to pass away:

  • Telephone 111 to notify them of the situation and they will arrange to visit to verify your loved one’s passing.
  • Once the doctor or locum has visited and verified the death, and you & the rest of the family are ready for us to convey your loved one into our care, no matter what time of day or night, please telephone us on 01455 637457.

It's important to remember that there is a six hour window for the doctor or locum to attend from the initial call being placed.

  • When you are ready to call us, we will take some details and then arrange for our team to attend and respectfully convey your loved one into our care.
  • It is then necessary for Medical Certificate of Cause of Death (commonly referred to as the death certificate) to be forwarded the Medical Examiner’s Office by the GP Practice
  • The Medical Examiner is a senior medical practitioner and will review your loved one’s notes and medical history before telephone you to discuss the probable cause of death
  • This will give you the opportunity to either agree with the cause of death or raise any issues or concerns you may have before the certificate is electronically forwarded onto the Registrars in the sub district of where your loved one has passed away. The M.E.O will advise if the registrars contact you, or if you will need to contact them to arrange a face-to-face appointment. 
  • Should the Medical Examiner make a referral to H.M. Coroner for further investigation you will be notified.

Passed Away at a Care Home

The next steps to take if your loved one has been poorly and was expected to pass away:

  • The care home will arrange for a doctor or locum to attend to verify the death.
  • Once the doctor or locum has visited and verified the death, and you & the rest of the family are ready for us to convey your loved one into our care, please telephone us on 01455 637457.

  • It is then necessary for Medical Certificate of Cause of Death (commonly referred to as the death certificate) to be forwarded the Medical Examiner’s Office by the GP Practice
  • The Medical Examiner is a senior medical practitioner and will review your loved one’s notes and medical history before telephone you to discuss the probable cause of death
  • This will give you the opportunity to either agree with the cause of death or raise any issues or concerns you may have before the certificate is electronically forwarded onto the Registrars in the sub district of where your loved one has passed away. Passed Away in Hospital

The next steps to take if your loved one has been poorly and was expected to pass away:

  • The Medical Certificate of Cause of Death (commonly referred to as the death certificate) to be discussed with the Medical Examiners Office by the Ward Doctor or Consultant.
  • This will give you the opportunity to either agree with the cause of death or raise any issues or concerns you may have before the certificate is electronically forwarded onto the Registrars in the sub district of where your loved one has passed away. The M.E.O will advise if the registrars contact you, or if you will need to contact them to arrange a face-to-face appointment. 
  • The Medical Examiner is a senior medical practitioner and will review your loved one’s notes and medical history before telephone you to discuss the probable cause of death
  • Should the Medical Examiner make a referral to H.M. Coroner for further investigation you will be notified. 
  • In the majority of hospitals, we cannot convey your loved into our care until the required paperwork is in place and we have been given permission by the hospital. Please be assured we will do everything possible to bring your loved one into our care as soon as possible.
  • The hospitals where we can convey your loved one into our care after their death has been verified is Hinckley & Bosworth Community Hospital, Coalville Community Hospital and St. Luke’s Hospital, Market Harborough. This isn’t an exhaustive list, please contact us if you are unsure.

Passed Away in a Hospice

The next steps to take if your loved one has passed away in a hospice:

  • The Medical Certificate of Cause of Death (commonly referred to as the death certificate) will be discussed with the Medical Examiners Office by the Doctor.
  • This will give you the opportunity to either agree with the cause of death or raise any issues or concerns you may have before the certificate is electronically forwarded onto the Registrars in the sub district of where the hospice is located. The M.E.O will advise if the registrars contact you, or if you will need to contact them to arrange a face-to-face appointment. 
  • The Medical Examiner is a senior medical practitioner and will review your loved one’s notes and medical history before telephone you to discuss the probable cause of death
  • As soon as the paperwork is in place, the hospice will give us approval to bring your loved one into our care. Please be assured we will do everything possible to bring your loved one into our care as soon as possible.

    When you have been notified that the form has been emailed to the registrar, an appointment with the registrars in the area where the hospital is located will need to be arranged. 

Passed Away Unexpectedly

  • When someone passed away unexpectedly, it is normal for the Police to attend. They are there on behalf of the coroner to collect information.
  • Your loved one will be conveyed to a local hospital, into the care of the coroner.
  • Please contact us on 01455 637457 when you are able, we will liaise with the coroners office to make sure we can bring your loved one into our care as soon as possible. We will be on hand to support you and guide you every step of the way.
